Intraoperative Complications in Keratorefractive Lenticule Extraction (KLEx)

Since the all-femtosecond laser approach in keratorefractive lenticule extraction (KLEx) is a fundamentally different approach of laser vision correction, the comparison of intraoperative complications (rates) to longer existing procedures like laser-assisted in situ keratomileusis (LASIK) or photorefractive keratectomy (PRK) is critical. The most common appreciable complications in KLeX are suction loss (ranges from 0.9% to 4.4%), black spots (ranges from 0.33% to 11%), and difficulties in lenticule extraction (ranges from 2% to 9%). Cautious preoperative preparations, confidence with the “rescue-programs” of the laser platform and surgical experience could lower the rates of complications significantly. Minor complications like epithelial defects (ranging from 0.2% to 11%), tears in the cap (ranging from 2.09% to 11.05%), or cap perforations (up to 4%) are most often not affecting the final visual acuity negatively. However, even for novel surgeons, the learning curve seems to be relatively short to achieve similar results as in other keratorefractive laser treatments.
