Small incision lenticule extraction (SMILE) has become an established procedure for the correction of myopia. Suction loss during the procedure prevents the femtosecond cutting of the lenticule interfaces from completing. In this chapter, we describe the different management options for suction loss by either continuing with SMILE, converting to LASIK or surface ablation, or aborting the procedure. Most cases of suction loss result from a patient’s eye movement, which led to the understanding that managing patient comfort and cooperation was paramount in minimizing the incidence of suction loss. Advancements in technology led to the introduction of the VISUMAX 800 which has a faster lenticule delineation time and is expected to further reduce the incidence of suction loss.
