Implantation of an intraocular lens (IOL) that offers full refractive correction at all distances is the ideal goal for cataract and lens-based refractive surgery. One of the possible attempts to achieve this goal is the implantation of multifocal IOL (MIOL). Adequate patient selection is essential to ensure a satisfied patient after the surgery. Dissatisfaction after multifocal IOL implantation may be reported by patients who do not achieve the desired visual goals, have limited visual acuity, or have new visual aberrations, with the most common complaint being blurred vision and photic phenomena. In this chapter, causes leading to patients’ dissatisfaction are discussed, e.g., IOL decentration, IOL tilt, inadequate (small) pupil size, residual refractive error, posterior capsule opacification, photic phenomena, reduced contrast sensitivity, and dry eye syndrome. Moreover, various possibilities of managing these clinical situations are covered.
