Corneal Inlays: Complications

Corneal inlays are utilized as a minimally invasive alternative to correct presbyopia. This chapter outlines the evolution, types, and complications associated with corneal inlays. Initially, inlays were thick and non-biocompatible, leading to various complications. Modern inlays are thinner, more permeable, and biocompatible, which has improved their safety and efficacy. There are three main types of inlays: pinhole optics (e.g., Kamra™), refractive inlays (e.g., Presbia Flexivue Microlens™), and those that reshape the corneal curvature (e.g., Raindrop™ Near Vision Inlay). While these inlays offer significant benefits, complications can arise, including reduced visual acuity, difficulty reading in dim light, loss of contrast sensitivity, optical aberrations, and issues related to pupil size. Despite advancements, challenges such as corneal haze and dry eye persist, highlighting the importance of careful patient selection and precise surgical techniques for successful outcomes.
