Extent of Resection in Low-Grade Gliomas: Surgical Considerations, Importance of Brain Mapping, and the Role of the Molecular Signature of the Tumor

Supratentorial diffuse low-grade gliomas (DLGGs), classified as World Health Organization (WHO) grade II, are diffuse neuroectodermal primary brain tumors [1, 2]. They are characterized by continuous and relatively slowly growth at an average rate of 4 mm/year [2, 3]. Their diffuse infiltrative pattern, tumor invasiveness, and migration along the white matter tracts limit extent of surgical resection, jeopardizing the efficacy of oncological treatments. Most of those tumors will transform to a higher grade of malignancy at some point during follow-up, leading to neurological disability and, potentially, to death [2–7].
