In many cases, a second candidate will be recruited in parallel as a back-up in case the primary recruitment is not successful. At the point of being recruited, if not sooner, you should be fully transparent with your medical school about your status, including why you are considering another position and, if asked, whether you would consider a retention offer. You should avoid playing outside offers and retention offers against each other. Upon accepting a final negotiated offer, you should work with both institutions to craft appropriate announcements and clarify their expectations of your responsibilities during the transition to make it as smooth and successful as possible.
