Treatment Maneuvers for Benign Paroxysmal Positional Vertigo (BPPV)

The underlying mechanism of benign paroxysmal positional vertigo (BPPV) is that otoliths break loose, exit the otolith organ, enter one or more semicircular canals, and erroneously stimulate the cupula of the semicircular canals, either directly (by impinging on the cupula) or indirectly (by influencing endolymphatic fluid dynamics). Treatments for BPPV involve moving the head through a series of positions, leveraging gravity to move the otoliths out of the semicircular canals and into the utricle, where the otoliths will eventually get resorbed by dark cells. The maneuver (series of head positions) used to treat BPPV depends on which canal (or combination of canals) is involved.
