Benign paroxysmal positional vertigo (BPPV) involving the lateral (horizontal) semicircular canal is the second most common type of BPPV. Lateral canal BPPV can be diagnosed by identifying geotropic or apogeotropic direction-changing positional nystagmus on the supine roll test. Usually, physical examination alone does not clarify the affected side, so vestibular rehabilitation therapy often takes an empiric approach. Treatment maneuvers include the Lempert roll, Gufoni maneuver, or Appiani maneuver.
