The Valsalva maneuver can provoke nystagmus in some patients with otologic diseases such as perilymphatic fistula or third window phenomena. The mechanism is thought to be that Valsalva-induced intracranial pressure changes can get transmitted to (and through) the inner ear, pathologically stimulating the vestibular system.
