Loxoscelism is common in various regions of the world and in Latin America. It is caused by Loxosceles intermedia, L. gaucho, and L. laeta and the L. reclusa in North America. The venom produces dermonecrosis and eventually systemic manifestations, such as hemorrhages, hemolysis, and rhabdomyolysis, which can lead to acute kidney injury (AKI). The main toxins in the venom are phospholipase D, hyaluronidase, serine protease, and metalloproteinases with great nephrotoxic power and which produce activation of the inflammatory phenomenon, hemodynamic disorders with alteration of the microcirculation, hemolysis, rhabdomyolysis, and direct tubular and glomerular nephrotoxic action. Kidney involvement is particularly severe in children, in whom hemolytic anemia is observed in half of the cases, rhabdomyolysis in a third of them, and AKI in 12%. Antivenom can be useful in some cases.
