This chapter focuses on technical aspects to set up a project for orthognathic surgery. Unlike other procedures of virtual surgical planning (VSP), which require more technical expertise, VSP in orthognathic surgery is a highly standardized process, and this is due to its wide adoption in clinical practice, as well as the availability of dedicated software for VSP. The correct sequence of steps from segmentation, intraoral scanning alignment, cephalometry, osteotomies, occlusion transferring, and the sequence of repositioning is elucidated to structure a protocol useful for clinicians. Moreover, this chapter focuses on innovations for such aspects, introduced in a modern software for orthognathic surgery (Enlight CMF, Materialise NV, Leuven, BE), the most important of which being virtual occlusion, whose applicability is discussed. Last, perspectives on how artificial intelligence (AI) is expected to introduce a shift in treatment plans for orthognathic surgery are presented, with definition of current evidence.
