Graving dock XYZ is one of the work areas in the shipyard owned by PT. ABC has a high fire risk due to flammable materials and hot work processes such as cutting steel plates and pipes, grinding, and welding. The hot work is carried out simultaneously in the same work area. Fires are one of the most common operational obstacles in shipyards. The study aims to determine the cause of the fire in Graving Dock XYZ. This study uses the Failure Modes, Effect, and Criticality Analysis (FMECA) method to find priority risks, analyze equipment or production processes, identify failure types, and determine the consequences of these failures, including identifying critical points. The Fishbone Diagram is used to find the root cause of these priority risks. In this study, seven potential fire risks can occur in Graving Dock XYZ, and there was one risk at the high-risk level, namely the burning of existing plate coating during hot work.
