An average of seventeen years elapses before health intervention research translates from laboratories to real-world settings where benefits can be realized by the people for which they were designed, if such translation even occurs at all. Digital health interventions face additional barriers beyond those of traditional health intervention research. These barriers result from the additional complexity of digital health research given the nature of rapidly changing software, technologies, and systems upon which it relies and that must be integrated and tested at every research stage. However, there are few guiding resources for the systematic planning and development of digital health interventions from earliest project ideation to translation of effective interventions in real-world implementations. To close this gap, the goal of this report is to: 1) identify specific barriers to digital health research translation, 2) describe the Translation By Design (TBD) framework for digital health research, and 3) highlight a progression of proposed studies that completes with a translated real-world implementation based on a use case for an Augmented Cognition digital health intervention that supports informal caregivers of older adults at risk for falls.
