Home-based primary care is a healthcare delivery model that allows physicians to bring comprehensive, interdisciplinary medical care to patients with complex chronic conditions and/or mobility issues in their homes. This model has been shown to reduce healthcare costs, reduce hospitalizations, and improve patient outcomes. A team may consist of physicians, nurse practitioners, nurses, social workers, and other healthcare professionals who work together to provide coordinated care. This chapter will discuss the history of home care, the different types of models, and the future directions in which this type of healthcare model will continue to grow.
