Optimizing the Design and Usability of Digital Weight Loss Patient Decision Aids: Insights from Expert Interviews

With the rise of digital healthcare, digitizing patient decision aids (PDAs) can enhance both efficiency and accuracy in medical decision-making. However, creating intuitive, user-friendly, and effective digital tools remains a challenge. This study focused on a weight loss PDA (OptiWeight), transforming a paper-based tool into a web-based platform optimized for iPad use. Guided by the Consolidated Framework for Implementation Research (CFIR), we conducted face-to-face interviews with 15 weight management experts (10 attending physicians, 1 nurse/case manager, and 4 dietitians) to identify user needs and potential barriers to clinical integration. All interviews were audio-recorded, transcribed verbatim, and analyzed via open coding under CFIR domains. Experts then completed the System Usability Scale (SUS) and NASA Task Load Index (NASA-TLX). Overall, participants provided positive feedback (M = 8.2, range 1–10), citing practicality and acceptability. Key suggestions included simplifying tutorials, enlarging fonts, optimizing chart designs, and adding pairwise comparison features. The SUS score of 77.8 (range 1–100) was classified as ‘Good’ usability, while a NASA-TLX score of 7.16 (range 0–20) reflected moderate mental workload, suggesting room for improvement. We conclude that the OptiWeight was well-received by experts but requires ongoing refinements to reduce cognitive load and enhance user experience. Such optimizations may ultimately increase patient engagement in weight loss decision-making, support more effective weight management, and offer healthcare professionals a more efficient tool for patient education and communication.
