Mastopexy with the Inclusion of Implant Using Lower Dermal Flap

Introduction: Augmentation mammoplasty is the most popular surgery of the specialty worldwide. Conflicting objectives coexisting in the same surgery, as occurs in breast augmentation associated with mastopexy, with an increase in breast content and skin reduction, imply several complications and reoperation rates. This antagonistic competition can lead to disastrous tissue failure to cover the implant, increasing its risk of exposure, infection and biofilm formation. Method: The clinical examination should focus on the patient’s body constitution to ensure that breast augmentation is proportionate and harmonious. The flap is designed by marking the submammary fold and the area immediately above it. We performed a de-epithelialization of this region, raising the dermal flap that will support and reinforce the support of the implant, which is placed in the subfascial plane. Discussion: Whenever possible, it is recommended to opt for smaller implant volumes, thus minimizing an early fall and other possible complications. In this way, the technique can provide pleasant aesthetic results that tend to be maintained in the long term. Conclusions: The inferior dermal flap technique contributes to stabilizing the implant through an increase in scarring fibrosis in the sulcus, with a resultant local tissue stiffening and reinforcement of breast support, in addition to increasing the protection of the prosthesis in cases of complications such as breast dehiscence, surgical wound, or even in the face of skin suffering/necrosis.
