Esophageal Diverticula

Esophageal diverticula may occur in any segment of the esophagus. Pulsion-type diverticula are more common and result from over-pressurization of the esophageal lumen. The increased pressure causes progressive protrusion of the esophageal mucosa and submucosa through anatomically weak areas within esophageal musculature. Traction-type diverticula often relate to scar tissue from prior surgery or inflammatory conditions pulling on the esophagus. Dysphagia is the primary symptom, but regurgitation, recurrent cough, halitosis, and aspiration may also be present. Epiphrenic diverticula, emerging from the distal esophagus, are often related to a spastic or hypercontractile disorder of the esophagus. Contrast-enhanced esophagram typically clinches the diagnosis, but complementary upper gastrointestinal endoscopy and/or esophageal manometry may be necessary. Surgical treatment is reserved for symptomatic diverticula and may be completed by several different surgical and interventional subspecialties.
