Inflatable penile prosthesis (IPP) is the gold standard surgical treatment for medication-refractory erectile dysfunction (ED). While IPPs are generally safe and effective, a recent systematic review showed an overall device survival rate of 53% at 20-year follow-up. Among patients who undergo IPP revision, studies report mechanical failure rates of 50–69%. Inability to deflate penile prosthesis cylinders is an uncommon but significant complication which can occur as a result of overfilling the system, or due to capsule formation around the reservoir in patients who have not deflated their cylinders for a prolonged period of time (especially in the early postoperative period). Additional factors that can contribute to difficulty deflating a device include kinked tubing or obstruction of tubing from blood products or crystallization. Difficulty deflating the cylinders can lead to undesired protrusion of the penis, and can cause patient bother. We present such a case and describe the technique used to correct this form of device malfunction.
