This chapter introduces an innovative approach to managing complications associated with penile prosthesis implantation, particularly in patients with severe intracorporal fibrosis, often referred to as “woody induration.” This condition complicates implant procedures due to issues such as corporal perforation, cylinder crossover, infection, and urethral injury. The obturator fenestration technique is a method to help prevent and manage complications such as crossover, septal perforation, and others, by creating necessary space within the fibrotic corpora cavernosa, allowing for safe prosthetic implantation. Key advantages of this approach include reduced operative time, minimized risk of organ or vascular injury, and a lower likelihood of revision surgery.
