As the use of EEG has become more commonplace and younger preterm neonates have been studied more frequently, it has become apparent that electrical brain activity develops according to conceptional age, not actual age. EEG baseline background activity changes as the neonate matures and advances accordingly with gestational age. The most extreme premature babies do not show a distinction between wakefulness and sleep. Their background pattern is discontinuous pattern with no state change or reactivity until about 30 WGA. As the baby matures, clear patterns of waking, active sleep, and quiet sleep can be seen. Maturation is thus a continuum, and obtaining an estimate of a neonate’s gestational age can be made within a 2-week range based on EEG alone. This chapter outlines the typical patterns and findings in neonates based on their gestational age.
