Idiopathic facial aseptic granuloma (IFAG) is a self-healing benign inflammatory condition typically affecting the face of pediatric patients. IFAG manifests as well-demarcated, firm, erythematous to violaceous nodules or plaques on the central face, particularly the cheeks and eyelids. The most common dermatoscopic features include focal white structureless areas, followed by blurred linear vessels with branches and focal orange structureless areas. However, only the combination of focal orange and white structureless areas is considered characteristic of IFAG when compared with main clinical differential diagnoses. Notably, the absence of these features does not rule out the diagnosis of IFAG as they are not present in all instances.
