Annular lichenoid dermatitis of youth (ALDY) is a lichenoid skin condition clinically characterized by one or more asymptomatic red-violaceous, round to oval macules and patches, which often develop into annular lesions with an inflammatory, non-scaling border and occasional central hypopigmentation. As lesions age, the peripheral erythema usually fades, resulting in complete or incomplete rings of hyperpigmentation surrounding whitish, non-atrophic, and non-indurated areas. The main dermatoscopic clue of ALDY includes the presence of brown-gray dots, observed in around two-thirds of cases. Despite its diagnostic value, the absence of these dots does not exclude ALDY, underscoring the need for comprehensive clinical and histological correlation.
