PSMA-PET in Prostate Cancer

Prostate-specific membrane antigen (PSMA) PET imaging has demonstrated potential in addressing prostate cancer diagnostic difficulties. The FDA has approved three tracers for PSMA PET/CT studies: 68Ga-PSMA-11, 18F-DCFPyL, and 18F-rh-PSMA-7.3. According to current guidelines, 68Ga-PSMA PET is used as an adjunct to traditional imaging modalities in patients with primary prostate cancer. Besides the role of PSMA PET/CT in the initial diagnosis of prostate cancer, its performance in TNM staging and its correlation with prostate-specific antigen (PSA) levels are discussed in this chapter as well, based on meta-analyses. For N and M staging, PSMA PET/CT is more sensitive and specific than conventional imaging; however, for T staging, multiparametric MRI provides better accuracy. Beyond tracers with marketing authorization, some additional fluorinated PSMA tracers are discussed, such as 18F-PSMA-1007. Additionally, own patient cases are presented.
