Multicenter Trials of Prostate Radioligand Therapy (PRLT) in Castration-Resistant Cancer

The World Association of Radiopharmaceutical and Molecular Therapy (WARMTH) community reported two retrospective trials using Lu-177-PSMA-617 (the same compound as Pluvicto®), both published in the European Journal of Nuclear Medicine and Molecular Imaging. In these studies, there were more than 400 patients treated, and factors predicting outcome and excellent results could be confirmed before official registration trials, using real-world evidence data. The third WARMTH study with 225Ac-PSMA RLT was a continuation of the WARMTH multicenter trials: on the basis of a retrospective analysis with a wide clinical experience, multiple factors associated with overall survival and progression-free survival for 225Ac-PSMA RLT were found. Additionally, I was requested to provide a commentary for The Lancet Oncology where this work was published. The fourth multicenter study presented in this chapter suggests that 177LuPRLT is a promising therapy for mCRPC patients with lymph node-predominant metastasis, providing significant PSA response and prolonged survival with manageable side effects.
