Prevalence and Drug Susceptibility of ESBL-Producing Escherichia coli in Infected Patients at Can Tho General Hospital

The prevalence of antibiotic resistance in gram-negative bacteria, notably Escherichia coli (E. coli), presents a significant healthcare challenge. This resistance is frequently attributed to the synthesis of β-lactamases, especially the extended-spectrum β-lactamases (ESBLs). The research aimed to detail the resistance patterns and the occurrence of ESBL-producing genes in E. coli strains collected from patients with infections at the general hospital in Can Tho City. During sixteen months, a total of 258 E. coli were collected from 1248 infectious patients. Susceptibility testing showed high resistance of E. coli to several antibiotics, with the most resistant being ampicillin at 98.8%. This was followed by resistance to Ampicillin/Sulbactam (83.3%), levofloxacin (82.9%), ciprofloxacin (79.8%), cefazolin (78.7%), and cefepime (74.4%). Among the 258 isolates, 143 produced ESBLs. The Multiplex PCR analysis revealed that the predominant ESBLs were CTX-M, present in 73.4% of cases, with TEM and blaSHV following at 39.2% and 9.8%. In the CTX-M gene group, 3 genotypes were identified: blaCTX-M-1 (60.0%), CTX-M-2 (38.1%); blaCTX-M-9 (1.9%). Genetic analysis showed that multidrug resistance was statistically significantly associated with ESBL-producing genotypes, a combination of ESBL-producing genotypes (p < 0.05).
