Approximately 98% of MBC are invasive carcinomas of no special type compared with 80% of FBC. Invasive lobular cancer is responsible for 2.5% of male cancers against 10.5% in females and two thirds of those affected males have other risk factors such as Klinefelter’s syndrome and BRCA1/2 mutations. Pleomorphic lobular carcinomas are very rare and their potential aggressiveness in males is unclear. The significance of ADH in risk of MBC is at present undetermined. Papillary lesions have mostly been reported as single cases: benign, DCIS or invasive with micropapillary with the latter carrying a high risk of early metastasis. Secretory cancers mostly occur in males aged <20 years and almost always there is a ETV6-NTRK3 fusion. Adenoid cystic carcinoma is also rare and, if operable, requires aggressive local treatment. Mucinous carcinoma carries a reasonable prognosis despite being diagnosed in older men. Breast sarcomas in males have a similar distribution of subtypes as in females but are very uncommon. Sarcoma surgery strives to achieve local clearance and avoids nodal surgery unless there is direct invasion.
