The extended-view totally extraperitoneal (eTEP) access approach was conceived to overcome the limitations of the traditional totally extraperitoneal technique for the repair of inguinal hernias. The eTEP approach has the advantages of facilitating the implementation of the steps of the Critical View of the Myopectineal Orifice and extending the indications of the extraperitoneal approach to hernias previously repaired via other approaches. eTEP access permits the expansion of the usable extraperitoneal space, which is achieved by dividing its natural boundaries, using a flexible and ergonomic port setup, and placing meshes in a retromuscular position with limited fixation. It provides a new approach for the repair of ventral, incisional, and lumbar hernias.
