Atypical hernias are a group of hernias that are challenging to repair using traditional laparoscopic and open methods due to their proximity to bony structures and sensitive structures that preclude safe and easy fixation of mesh and proper overlap of mesh beyond the borders of the defect. The robotic platform gives the surgeon the ability to overcome these traditional shortcomings of inadequate mesh overlap and difficult mesh fixation and provides flexibility in the repair of hernias in atypical locations. This chapter details the three common approaches—rTAPP, TARM, and eTEP RS—and describes the use of the preperitoneal and retromuscular compartments of the abdominal wall for placement of mesh between well vascularized tissue outside the peritoneal cavity.
