Stapled Endo-laparoscopic Retro-Rectus Repair: The THT Technique

In recent years, the push for innovation in abdominal wall surgery has increasingly brought to the fore the classic Rives Stoppa repair, reproduced with endoscopic or laparoscopic techniques. The aim is to combine the advantages of a solid sublay mesh reconstruction with those of mini-invasive surgery. Trentino Hernia Team (THT) fits into this context as an endo-laparoscopic technique that uses a single umbilical port to access the retromuscular space and linear staplers for suturing the rectal sheaths, making the abdominal wall reconstruction fast and safe, minimizing the surgical trauma to the patient. This chapter exposes the rationale, history, advantages, limitations, and critical points of the technique. A step-by-step illustration of the procedure will detail what to do and what not to do to reproduce THT successfully.
