This chapter initially outlines current shortcomings in the indexed literature concerning terms used to name abdominal wall planes. Thereafter, it highlights the most important classification systems that have been published, focusing on those that have, to date, attempted to standardise terms. We explain our attempt at standardisation and describe and discuss our Delphi exercise that led to ICAP, an ‘International Classification of Abdominal Planes’. The 10 ICAP planes are called ‘onlay’, ‘anterectus’, ‘retrorectus’, ‘retromuscular’, ‘interoblique’, ‘retrooblique’, ‘inlay’, ‘transversalis fascial’, ‘preperitoneal’, and ‘intraperitoneal’ and are illustrated in detail. Lastly, we examine the literature and interpret current evidence around optimal plane of mesh placement for specified clinical scenarios.
