Open Anterior Component Separation and Onlay Repairs

The onlay ventral repair is noted for its technical simplicity, shorter surgical time, and a lower learning curve, despite historical concerns about surgical site occurrences (SSOs). Recent data indicates that, when performed correctly, onlay repairs can yield results comparable to sublay techniques. Key principles include anatomical restoration prior to mesh placement, meticulous defect closure, and the use of appropriate suture materials and other strategies to enhance tissue union and avoid seroma. Anterior component separation techniques are considered an extension of the onlay repair, particularly for large or complex hernias, to allow abdominal wall closure in larger hernias.
