Upper Extremity Tendon Transfers

Tendon transfer, a surgical technique utilizing functional muscle-tendon units to restore lost motor function, has historical roots traced back to Missa’s work in 1770. The late nineteenth century witnessed a surge in research, particularly during the polio epidemic, with Nicoladoni’s peroneus longus tendon transfer and Drobnik’s treatment for upper extremity paralyses serving as early milestones. Indications for tendon transfers encompass severe peripheral nerve injuries, brachial plexus injuries, and conditions like rheumatoid arthritis or infectious agents. Preoperative considerations involve evaluating trauma history, conducting electrophysiological studies, and considering factors influencing recovery. There are important points to be considered and adopted as general principles for successful tendon transfers. Fundamental principles include ensuring healthy soft tissue coverage, adequate joint mobility, and avoiding critical loss of function from the donor tendon. Attention to muscle strength, excursion, and optimal tension is paramount for successful transfers. The Pulvertaft technique, widely employed, entails passing the donor tendon through tunnels in the recipient tendon. Alternative methods like the spiral technique and side-by-side tenorrhaphy offer diverse approaches. In cases where primary nerve repair is not possible or findings related to reinnervation cannot be obtained after nerve reconstruction, and sufficient improvement in muscle functions cannot be achieved, tendon transfers serve as reconstructive solutions aiming to reinstate lost motor function.
