Fractures and Dislocations of Hand Region

Hand fractures are a common type of injury, accounting for 18–44% of all musculoskletal fractures. The management of these fractures varies depending on the severity and location of the fracture. For metacarpal bones, open reduction and internal fixation are often indicated, particularly for fractures of the mid or distal regions of the bones, where displacement or axial malalignment is significant. Various operative and nonoperative treatment strategies have been proposed to achieve anatomic restoration, fracture stability, and early rehabilitation, facilitating an early return to daily life. Phalanx fractures, particularly those involving the proximal and middle phalanges, are common in various populations, including children and individuals involved in sports activities. Classifications of phalanx fractures are based on localization, shape, angulation, or associated injuries, with intra-articular fractures posing movement restriction risks. Standardized treatment is challenging due to various factors, and the goal is to achieve union in an anatomical position with full joint motion. The treatment often involves percutaneous pinning and careful consideration of radiographic configuration, healing processes, and potential complications. Understanding the specific characteristics and treatment approaches for phalanx fractures is crucial for optimizing patient outcomes and ensuring effective rehabilitation. Dislocations of the carpometacarpal joints can easily be missed due to the subtle nature of radiographic findings. They are rare injuries but may have significant effects in hand function particularly in grasping and pinching. Correct diagnosis necessitates a thorough understanding of the normal radiographic anatomy of the carpometacarpal joints. Injuries to the proximal interphalangeal joint (PIP) are among the most prevalent in the hand, often underestimated in severity. These injuries can result in extended disability, pain, and stiffness. In this chapter, we will discuss the prevalence and management of fractures and dislocations of hand considering the varying severity and location of the fractures. We will explore the indications for surgical intervention, including open reduction and internal fixation, as well as nonsurgical management options. Additionally, we will review the proposed surgical indications for each region and the various operative and nonoperative treatment strategies aimed at achieving anatomic restoration, fracture stability, and early rehabilitation.
