The Clinical Hand: How It Can Be Applied in Primary Care to Treat Substance Use Disorders and Behavioral Health

Most clinicians would agree that no two patient visits are the same. The complexities of diagnoses combined with holistic care can result in a variety of presentations and recommended plans. Navigating an office visit while considering the many aspects of a patient can feel difficult and unstructured. In patients with substance use disorders and behavioral health disorders, this may even be combined with deeply emotional content. In this chapter we will focus on the use of one tool, The Clinical Hand, that provides structure to an office visit in a way that expands clinicians’ relationship-centered care skills through building trust and connection with patients. Within the tool are five guideposts for office visit navigation and include connecting, setting an agenda, handing over information, safety-netting the visit, and housekeeping for clinician wellness. They will be defined and discussed using practical, reproducible examples. Along the way, additional skills will be introduced such as the BATHE technique, navigation of the biopsychosocial model, and how to use shared-decision making. At the conclusion of this chapter, the reader will be able to utilize this tool in their clinical practice, resulting in a comprehensive and structured office visit while strengthening relationships with patients.
