Understanding the Role of Nurse Case-Management to Improve Engagement and Community Access to Care

The knowledge base and clinical skill set of nurses and their deep understanding of the assessment and implementation process make them successful case managers in the interdisciplinary field of substance use disorder (SUDs) and behavioral health. Symptoms of active SUDs and MH significantly impact all areas of a person’s health and wellness, which require attention during the journey of recovery. Navigating the systems of care needed to improve overall health is difficult for individuals facing healthcare disparities. Whether inpatient, outpatient, or community, nurse case managers help vulnerable populations meet their goals by promoting patient-centered care that will address each person’s needs. Specialized nurse care coordination fills the gaps left behind during transitions of care.
