Trigeminal neuralgia, characterized by severe facial pain along the maxillary and mandibular nerves, poses a significant clinical challenge. This chapter explores the role of ultrasound in understanding trigeminal neuralgia’s anatomy and guiding injections for both superficial and deep nerve blocks. Within superficial nerve blocks, the chapter delves into the anatomy and clinical significance of the supraorbital, infraorbital, and mental nerves, detailing ultrasound-guided injection techniques that emphasize precise localization for optimal outcomes. For instance, the in-plane approach with power Doppler imaging aids in targeting the supraorbital nerve emerging through the supraorbital notch. Transitioning to deep nerve blocks, the chapter covers the sonoanatomy of the maxillary nerve, emphasizing the importance of precision in needle placement, facilitated by ultrasound, electrostimulation, and fluoroscopy. Similarly, the examination of the mandibular nerve focuses on anatomy and injection guidance, with a precautionary emphasis on avoiding maxillary artery injury using power Doppler imaging.
