During the last 10 years, injection of botulinum toxin A (BoNT-A) into the head and neck muscles has been used as an established and approved treatment method for chronic migraine. It has been shown to inhibit the release of acetylcholine and local nociceptive peptides at the terminal nerve endings. As the highest concentrations of nociceptive fibers are located at the cranial sutures, administration of BoNT is suggested to be performed on the suture lines rather than on the head and neck muscles in the treatment of chronic migraine. BoNT is quickly absorbed from the nerve endings in the sutural areas, and it then reaches afferent nerve fibers of the dura mater through the orthodromic and antidromic pathways. Accordingly, in this chapter, cranial suture injections for chronic migraine will be illustrated. Of note, the use of ultrasound guidance for targeting the cranial sutures is certainly expected to be safer and more cost-effective as well as to provide better pain relief in chronic migraine.
