Lateral epicondylitis is the most common condition affecting the elbow and is self-limiting in most cases. However, when conservative treatment fails, surgery is required. Ultrasound-guided extensor carpi radialis brevis (ECRB) release was performed with the patient in the supine position and office procedure under WALANT anesthesia. We start the procedure by first repairing the structures at risk: the lateral collateral ligament and the posterior lateral cutaneous nerve. The section starts from lateral to medial and then the tendon is sectioned and its aponeurosis with a Kemis® device. A minimally invasive approach using the ultrasound (US) has been already described with a complex technique allowing a full release of the ECRB. However, it is known that a simple aponeurosis section is sufficient for treatment as it has been shown by arthroscopic and open series. We proposed a simplified technique easily reproducible and seems to give good results with a faster recovery and without major complications.
