Carpal Tunnel Syndrome (CTS) is widely recognized for its neurological implications but less so for its dermatological manifestations. This case highlights an 89-year-old woman presenting with erosive, non-healing lesions and nail abnormalities in the fingers innervated by the median nerve. Clinical examination and diagnostic tests confirmed CTS with associated acroosteolysis and trophic changes in the skin and nails. The findings emphasize the importance of considering CTS in differential diagnoses of cutaneous and nail abnormalities, particularly when neurological symptoms coexist. Early recognition and management of these atypical manifestations are critical for comprehensive care.
