In this chapter we discuss the foundational principles that create the ethical framework for listing a patient for heart transplantation, optimizing donor & recipient risk matching, as well psychosocial and financial considerations. While each patient’s case must be evaluated on its own merits, the broader goal is to maximize the overall benefit of transplant programs and ensure the fair distribution of donor hearts.
