Following decades of experiments in animal models, the world’s first genetically modified pig-to-human heart transplant took place in January 2022. In this chapter, we will discuss several considerations and challenges in xenotransplantation, including anatomic and physiologic differences between pig and human hearts: immunological response and rejection, organ overgrowth, infections, and ethical dilemmas. We also describe the clinical course of the first genetically modified heart xenotransplant.
