There is growing consumers concern regarding consumption of food products that not only provide the main nutrients, but also provide of nutraceutical proprieties. In this sense, the organic products are increasingly favored as an option to get a healthy food consumption. The aim of this study was to compare conventional system (CS) and organic system (OS) dairy production using a systematic review and meta-analysis approach. An analytical dataset was constructed from 214 different studies. Results showed that animals managed under OS produce less milk (SMD −0.65; 95% CI; p = 0.0007) compared to those under CS. There were no significant differences in fat, protein, lactose, and monounsaturated fatty acids (MUFA) concentrations between OS and CS milk. However, OS milk had higher polyunsaturated fatty acids (PUFA; SMD +1.13; 95% CI; p = 0.005) and alpha-linolenic acid (ALA; SMD +1.54; 95% CI; p = 0 .0004) both accompanied by high heterogeneity (I2 > 93.0%). Also, we found that the content of conjugated linoleic acid (CLA) was higher in OS (SMD +3.06; 95% CI; p = 0.04) with a large heterogeneity (I2 > 94.0%). We conclude that based on the analyzed parameters, mainly the FA profile, the milk from OS can be a healthy option for human consumption. However, other milk characteristics such as residues (e.g. antibiotics) need to be considered in future studies to compare CS and OS systems. Finally, the identified differences between CS and OS production systems can mainly be explained by the animal species and the feeding management.
