Osteopathic Manipulative Medicine (OMM) offers a safe, non-pharmacologic approach to the management of rhinitis. Chapman reflexes (CRs) provide an integrated osteopathic approach to the diagnosis and treatment of lymphatic congestion of the nasal sinuses and surrounding structures. CRs were first described in the early twentieth century as localized, “neurolymphatic” tissue texture changes associated with visceral illness, disease, or injury. There is limited data on the mechanism of action or clinical effects of CRs, but a growing body of evidence illustrates the potential for Osteopathic Manipulative Treatment (OMT) to influence the lymphatic, sympathetic and parasympathetic nervous systems. Several animal and human studies suggest that therapeutic manipulation applied to the lymphatic system influences lymphocyte migration to important immune structures. Modulation of sympathetic and parasympathetic innervation levels associated with the cranium, ears, nose, and throat inhibit or augment drainage of these allergic sites. We define anterior and posterior Chapter points associated with rhinitis to aid in its diagnosis and treatment. We describe OMT application of CRs, selectively for diagnosis, or to enhance treatment, through direct rotatory stimulation or other OMT techniques for surrounding tissue. We outline a step-by-step comprehensive assessment in the identification of CRs and OMT options to effectively alleviate their associated lymphatic congestion. The palpable somatic tender points we recognize as CRs demonstrate a viscerosomatic relationship with the pathophysiology of rhinitis and may be targeted through OMT techniques. CRs offer a safe, non-pharmacologic approach to the management of rhinitis, as a complement to pharmaceutical and various other treatment options.
