Unlike the far more common vertebral body metastases, intradural pathologies seldom compromise spinal column biomechanics in and of themselves. Rather, biomechanics are altered by surgical resection of these lesions. One of the most common complications is postlaminectomy kyphosis, a progressive postoperative deformity that affects between 10 and 30% of adult patients but may affect over 50% of pediatric cases. In this chapter, we provide a brief overview of the posterior ligamentous complex and its contributions to spinal stability. Subsequently, we describe the results of prior clinical series describing the occurrence of postoperative progressive deformity following resection of intradural tumors, highlighting risk factors for this complication. We conclude by examining potential protective strategies highlighted in the literature.
