The Effect of Covid-19 and Systemic Sclerosis

The COVID-19 pandemic has posed unique challenges for patients with systemic sclerosis (SSc), a chronic autoimmune disorder characterized by fibrosis, vasculopathy, and immune dysregulation. This chapter explores the shared pathophysiological mechanisms between COVID-19 and SSc, including vascular endothelial damage, immune activation, and fibrosis, highlighting their implications for disease progression and management. COVID-19 may exacerbate SSc activity, leading to complications such as interstitial lung disease (ILD) progression, Raynaud phenomenon, digital ulcers, and renal crises. Conversely, SSc increases susceptibility to severe COVID-19, with prolonged hospitalization and poorer outcomes in patients with ILD or other comorbidities. Management strategies involve balancing immunosuppressive therapy, addressing respiratory complications, anticoagulation, and tailoring antiviral and anti-inflammatory treatments. Vaccination is critical, though reduced immunogenicity in immunosuppressed patients requires consideration. This chapter emphasizes the importance of multidisciplinary care, telemedicine, and patient education in mitigating risks and improving outcomes for this vulnerable population amidst the pandemic.
