The agri-food supply chain encompasses multiple stages, including Production, Processing, and Distribution, and involves various stakeholders such as producers, processors, distributors, and consumers. Ensuring food quality, safety, and availability is critical to addressing growing consumer demands and meeting regulatory requirements for safe, authentic, and ethically sourced products. Effective traceability solutions are essential to prevent contamination, verify authenticity, reduce fraud, and enhance stakeholder accountability and trust. This paper introduces a blockchain-based solution integrating IoT and Self-Sovereign Identity (SSI) technologies to enhance transparency, security, and data integrity in agri-food supply chains. By leveraging blockchain’s immutable ledger, the system provides tamper-proof and verifiable traceability data. The proposed solution combines Ethereum for traceability and IPFS for decentralized data storage, ensuring data privacy and security. A Proof of Concept demonstrates the system’s feasibility, highlighting its potential to reinforce trust and accountability across the agri-food supply chain.
