Retinal Detachments Associated with Giant Retinal Tear

Another characteristic type of detachment is retinal detachment (RD), caused by giant tears. It usually affects young males. Giant retinal tears are full-thickness tears that cover more than 90° of the retina associated with vitreous detachment. Giant retinal tears, which may be associated with ocular trauma, high myopia, aphakia, pseudophakia, and collagen genetic mutations, cause 1.5% of all rhegmatogenous RDs. The vitreous gel adheres to the anterior leaflet of the retinal tear, and the posterior leaflet of the tear moves freely and can fold on itself. This finding is useful in differentiating giant retinal tears from retinal dialysis. In retinal dialysis, the vitreous adheres to the posterior surface of the retinal tear, so the retina is not very mobile.
