Clinical Assessment

Patients with a known diagnosis of IBD will require ongoing clinical assessment throughout their disease management. The format of assessment is likely to vary between different patients, considering their current disease state and presenting symptoms. Some departments may routinely measure and record disease assessment scores such as the Harvey-Bradshaw Index (HBI) or Ulcerative Colitis Disease Activity Index (UCDAI), whilst others may use their own assessment plans and/or tools. Ongoing use of assessment tools can be helpful in detecting improvement or deterioration in disease activity. Using a structured approach to any clinical assessment can help to ensure a thorough and accurate process. Obtaining an accurate medical history is the first and perhaps the most important aspect. This should be in-depth at the first consultation and can usually be briefer at subsequent assessments, to determine any changes in health status since the last appointment. A physical clinical examination may then follow, and it is helpful to choose a structured approach that will ensure an appropriate clinical assessment.
