Young people receiving paediatric IBD health care will require transition to an adult-oriented IBD care team during late adolescence. Essential principles for successful transition dictate that the process is planned, appropriately timed, and flexible. Many models of transition exist which focus on either healthcare-professional-driven approaches or patient-driven approaches. However, none of these illustrate the specific processes involved in implementing the various models. This chapter will describe one model of transition (the sequential model) and the practical application of that process within a healthcare organization.
