Individuals with serious mental illness (SMI), including schizophrenia-spectrum disorders and bipolar disorders, have a higher prevalence rate of Criterion A traumatic events and PTSD (post-traumatic stress disorder) compared to the general population. Despite the great need for individuals with SMI to access gold-standard treatments for PTSD, like cognitive processing therapy (CPT), there are numerous barriers. Lack of adequate clinical training and clinician adherence to inaccurate beliefs are some of the most often-cited barriers. This chapter aims to instill confidence that treating PTSD using CPT is safe and effective with clients with SMI. This chapter provides specific clinical tips, guidelines and recommendations, and relevant case examples, illustrating how to effectively implement CPT with this at-need and often-underserved population.
